DepEd Email Request Form

1. To enhance the existing media of communication, promote a sense of identity, and facilitate a culture of collaboration, all teaching and non-teaching personnel are required to request and use the assigned DepEd email accounts.

2. DepEd Email requests are as follows:

    a. New DepEd Email: for those employees without DepEd Email.

    b. Password Reset: for those who already have their DepEd email but forgot their password. A maximum of 3 resets are given per DepEd emails, after                 that, the request form shall be forwarded to Central Office ICT Services.

    c. Rename: for those employees whose DepEd email are misspelled or change marital status.

    d. Account Transfer: for those employees who transfer from one division to another.

3. Effective immediately, this division will have the following steps for DepEd Email requests:

    a. Fill out the form, depending on the type of requests. (See attached Forms)

        i. DEPEDOZAMIZ–OSDS–ICT–001: for New DepEd Email Account

        ii. DEPEDOZAMIZ–OSDS–ICT–002: for DepEd Email Password Reset

        iii. DEPEDOZAMIZ–OSDS–ICT–003: for DepEd Email Rename

        iv. DEPEDOZAMIZ–OSDS–ICT–004: for Transfer of DepEd Email to other division

    b. Submit the request form to OSDS Administrative Personnel Office for validation of plantilla item.

    c. Submitted form shall be forwarded to OSDS ICT Services Office for processing. The office shall send messages to the mobile number, personal email or         Facebook Messenger provided in the attached form regarding the details of request.

4. This office Reiterates that DepEd Email should be used in all the DepEd and government online and offline transactions. No using of Yahoo and Gmail             account. Regular opening of the DepEd Email is highly encouraged.

Pre-printing of Bio-metric Attendance Data

1. Check the date format, should be Day - Month - Year. If not, right-click the task bar date.

2. Click "Adjust date/time"

3. Click "Date, Time, & Regional Formatting"

4. Scroll down and click "Change Data Format"

5. Under "Short Date" drop down text box, choose "05-Apr-17"

6. Check the task bar date if format is changed to Day-Month-Year
